    POSITION SUMMARY – RN Case Manager – Hospice POSITION DUTIES – Manage a case load of 15 patients, 4-5 visits per day depending on area and acuity level of patient, complete charting same day. RN must be willing to perform all service level nursing (routine, GIP, continuous, and respite), must be willing to perform admission, SIA and death visits, must be licensed in WA and preferably OR state nursing board, have a reliable vehicle to cover a 60 mile service radius (mileage will be covered at IRS standard rate), clean background check in WA and OR, drug screening, Influenza/TB/Covid vaccination records, and valid auto insurance. MINIMUM REQUIRED QUALIFICATIONS
    • WA/compact RN license
    • AHA BLS
    • Active DL and Auto Insurance
    • 1+ years minimum of current Hospice Case Management experience
    PREFERRED QUALIFICATIONS – Acute/Med Surg experience LENGTH OF ASSIGNMENT – 8 weeks SHIFT / HOURS PER WEEK – 8:30AM – 5:00PM/M-F or alternative schedule would be Sunday-Thursday/40 hours guaranteed. Possible weekends and call. SYSTEMS – WellSky Consolo; HOPE START DATE – ASAP
