    POSITION SUMMARY – RN Case Manager – HospicePOSITION DUTIES – Admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient’s environment. This role coordinates care with a multidisciplinary team, participates in the hospice program’s quality assessment performance improvement program and conducts follow up visits as needed.MINIMUM REQUIRED QUALIFICATIONS –

    POSITION SUMMARY – RN Case Manager – Hospice POSITION DUTIES – Manage a case load of 15 patients, 4-5 visits per day depending on area and acuity level of patient, complete charting same day. RN must be willing to perform all service level nursing (routine, GIP, continuous, and respite), must be willing to perform admission, SIA and death visits, must be licensed in WA and preferably OR state nursing board, have a reliable vehicle to cover a 60 mile service radius (mileage will be covered at IRS standard rate), clean background check in WA and OR, drug screening, Influenza/TB/Covid vaccination records, and valid auto insurance. MINIMUM REQUIRED QUALIFICATIONS –
    • WA/compact RN license
    • AHA BLS
    • Active DL and Auto Insurance
    • 1+ years minimum of current Hospice Case Management experience
