POSITION SUMMARY – RN Case Manager POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization MINIMUM REQUIRED QUALIFICATIONS – AZ/compact RN license; BLS; 3 years of performing Case Management/Care Coordination in an acute hospital setting within the last 5 years; Experience with and proficient at Midas, InterQual, and Microsoft Office programs; Experience with discharge planning AND utilization review; Minimum of 3 years hospital experience; Recent experience with Interqual (within past 6 months); Experience working with an EHR (electronic health record) PREFERRED QUALIFICATIONS – Experience performing CM/CC in the Emergency Department LENGTH OF ASSIGNMENT – 13 Weeks SHIFT / HOURS PER WEEK – 0830-1700 plus every other weekend SYSTEMS – Cerner START DATE – ASAP
