The RN Care Manager will oversee a daily caseload of patients on the inpatient units with a variety of care needs. Will attend interdisciplinary rounds. Complete Initial assessments. Facilitates the development of a plan of care in collaboration with the patient, family, caregiver and health care team. Communicate consistently with medical teams and patient regarding progress of care needs related to post discharge care needs, such as: home health, inpatient rehab. Will interact with post acute care referral sources to establish a plan. Document and Place orders for home health, DME, nutritional needs within EPIC EMR.

Ability to read, write and communicate in English; Current registration as a Registered Nurse in the Commonwealth of Massachusetts; Bachelor of Science Degree in Nursing preferred, (external applicants); Certificate in Case Management or CPUM or specialty preferred; Demonstrates competency with a minimum of 3 years acute care experience within the past 5 years with broad clinical experience in a hospital setting or case management. Demonstrate recent knowledge/experience within past 4 years in Discharge Planning and Utilization review; Working knowledge of InterQual®, or equivalent system. Strong interpersonal and negotiation skills demonstrated by a positive attitude, pleasant, professional and cooperative demeanor, with patients, physicians, fellow employees, and insurance companies; Excellent organization and time management skills; Ability to work independently and effectively in a fast pace environment; Ability to work productively in a stressful environment and effectively handle multiple projects and changing priorities; Proficient computer skills with ability to utilize and integrate updated software systems into practice.

The Johns Hopkins Hospital – Nurse Case Manager – Main Hospital Campus Department: Oncology service. the candidate should preferably have some Oncology experience or extensive Med/Surg bedside experience, they would more easily assimilate into the oncology environment. Required: Active RN License, (2) years RN experience, BLS, Current in-patient Case Management experience; EPIC and post-acute facility referral platforms such as NaviHealth or AllScripts. (5) 8 hour shifts; 8:30 am – 5 pm – No weekends; no holidays; no call The Case Manager Role: – Coordinates care for a select caseload of patients from triage, care coordination during JHM management and safe transfer to home. Proactively communicates with payors to ensure approval of treatment plan and required resources. – Maintains knowledge of and responds to clinical system, and fiscal data related to specific patient populations. . Collaborates with the multi-disciplinary team, patient/ patient family to ensure effective and efficient transition planning. Coordinates transition to post-acute settings including but not limited to nursing facilities, acute inpatient rehab and/or local shelters. – Manages patients based on departmental procedures and maintaining clinical safety and quality – Enters and maintains patient information in all departmental systems, per operational guidelines Note: Please remember – Bedside Case Managers need a current/active CPR card. They do not need the Med Test since they will not have access to or pass medications to a patient
