POSITION SUMMARY – RN Care Manager POSITION DUTIES –

• Coordinate concurrent utilization management activities house-wide and is the main link between the patient, health care team, organization, and payors during the patient’s hospital stay.
• Ability to conduct an assessment of patient needs, identify resources required to transition patients to the next setting, and secure the necessary resources.
• Collaborate, educate, and lead an interdisciplinary team of professionals in daily care rounds/care conferences/care planning.
• Excellent communication skills in written, verbal, and electronic systems such as email or with software applications currently used with the hospital.
• Ability to work with community organizations to secure durable medical equipment, health insurance authorizations, arrange transportation.
• Participate in the length of stay of an inpatient hospital population.
• Knowledge of hospital reimbursement methods and quality measures applicable to the care management department.

MINIMUM REQUIRED QUALIFICATIONS –

• Associate or Bachelor degree-prepared RN
• ME/compact RN license
• BLS
• 3-5 years clinical experience in medical/surgical nursing, home care, hospital case management and/or previous utilization review experience
• Utilization review and discharge planning experience

PREFERRED QUALIFICATIONS – Certification by a nationally recognized case management or utilization review organization (CCM or ACM) LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – Variable shifts of either 8:30a-4:30p or four 10hr shifts that will start at 7:30am-5:00pm. POSITION SUMMARY – RN Care Coordinator POSITION DUTIES – Coordinates the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payors. Utilizes advanced clinical skills to facilitate the provision of care which includes the assessment, planning, intervention and evaluation principles of the nursing process. Promotes and evaluates the effective utilization of resources using current clinical knowledge, awareness of community services, and assuming a leadership role with the patient care team to achieve optimal clinical and resource outcomes. Evaluates and analyzes quality/utilization cases, admission/discharge criteria, observation/1-day stays, readmits in 30 days, Medicare and diagnosis related criteria.  Participates in outlier review on an ongoing basis. May be assigned to the Emergency Department. In collaboration with the patient/family, multidisciplinary team including physicians and payors ensure the patient’s progress along with emergency episode of care in an efficient and cost effective manner for assigned case load. Advocates for the patient and family throughout the entire episode of care. Maintains availability to the patient/family as a resource to facilitate communication among providers and to monitor services rendered. Remains involved until the patient is discharged or appropriately transferred to the acute care setting or an alternate level of care. Initiate appropriate clinical pathways based on admission diagnosis and to serve as a resource person for utilization management purposes. MINIMUM REQUIRED QUALIFICATIONS –

• CA RN license
