JOB SUMMARY: Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plan in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process. ESSENTIAL FUNCTIONS: Accurately performs patient assessments and identifies patient needs. Identifies and initiates appropriate nursing interventions. Provides patient and family teaching throughout the rehabilitation stay and prior to discharge. Provides care appropriate to condition and age of the patient. Works cooperatively and actively participates with other members of the interdisciplinary team. Participates in Continuous Quality and Performance Improvement (CQPI). Maintains positive relationships with all customers. Promotes rehabilitation nursing philosophy and scope of practice. Actively participates in team conferences. Performs timely and appropriate documentation relating to medical necessity in the medical record. Responsible for completion and revision of the Interdisciplinary Care Plan for each patient. Performs timely and accurate QI assessments. Provides a safe environment for patients, visitors and staff and complies with appropriate and approved safety standards. Supports the leadership for the hospital in a professional manner and acts as a role model. Other duties as assigned. Qualifications QUALIFICATIONS: Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program. Current state licensure as Registered Nurse. CPR/BCLS certification. Mandatory Reporter Certificates CRRN Preferred Minimum six months’ Medical/Surgical experience in an acute care setting preferred. Excellent oral and written communication and interpersonal skills

Coordinates management of care and ensures optimum utilization of resources, service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of functions of case management, utilization review and management and discharge planning. Provides ongoing support and expertise through comprehensive assessment, care coordination, plan implementation and overall evaluation of individual patient needs while ensuring patient preferences. Serves as a patient advocate through resource utilization, discharge planning and addressing the holistic needs of the patient.

Coordinating patient care across the continuum using assessment, care planning, implementation, coordination, monitoring and evaluation for cost effective and quality outcomes in a primarily inpatient setting. Responsible for facilitating members’ transfer from UNMH to Presbyterian Hospital when services are available. Responsibilities: -Coordinates and facilitates the transfer of patients from UNMH to Presbyterian Hospital using the transfer center process flows (Presbyterian Care Connection). -Once decision for admission is made, collects clinical information to determine patient’s level of care. -Collaborates with Presbyterian Care Connection the transfer of member. -Conducts an in-depth assessment which includes psychosocial, physical, medical, environmental and financial parameters. -Collaborates with Healthcare team to proactively develop, implement and document, treatment and discharge plan, with appropriate resources to address social, physical, mental, emotional, spiritual and supportive needs. -Collaboratively formulates, implements, coordinates, monitors, and evaluates strategies with the healthcare team to address care management issues for specific patients and disease processes. -Applies utilization review criteria to assess and document the appropriateness of admission, continued stay, level of care, and readiness for discharge; refers cases that do not meet criteria to designated Physician Advisor. -Promotes the appropriate use of clinical and financial resources in order to improve quality of care and patient/member satisfaction. -Performs cost-benefit analyses and refers to negotiations as needed, with non-contracted providers and vendors. -Generates reports, which demonstrate efficacy through direct cost-savings and outcome measures. -Actively participates in the development of clinical guidelines and pathways and incorporates processes into the role of nurse care coordinators. -Educates providers and other PHS/PHP departments on health management strategies and care coordination services. -Receives, reviews, verifies, and processes requests for approval of pre-service and concurrent services, supplies, and/or procedures, including but not limited to, inpatient hospitalizations, diagnostic testing, outpatient procedures and services, home health care services, durable medical equipment, and rehabilitative therapies. -Performs retrospective review and conducts on-site or desktop audits at provider locations within New Mexico and completes all documentation accurately and appropriately. -Upon completion of medical record validation and other retrospective audits, compiles detailed findings and relevant supporting documentation for review. -Performs other functions as required.
