Nurses are acutely in the middle of tx, day 0, day 1 and during the sickest time – unless they are being intubated they are on this unit ? o Will you accept a first-time traveler? Yes if they are a strong BMT RN ? Patient Types/Common Diagnoses: o Ages: Neonate (more rare) to adults (if patient had childhood cancer treatment here, they will come to unit, even as an adult) o “On-Service”: Solid tumor cancers, Leukemia, Sickle cell, Aplastic anemia ? Highly acute bone marrow transplant population – this experience and skillset is a MUST – will receive patients s/p day 0, day 1 and 2 ? If child does not require intubation, they will come to this unit o “Off Service”: Can take off service (non-Hem/Onc) but must be without communicable infections/precautions due to the neutropenic patient population o Behavioral Health: unit will also commonly see BH Peds patients – diagnoses to include but not limited to: Pediatric behavioral/adjustment disorders, Suicidal Ideation, elopement, human trafficking ? Unit is equipped with cameras in care areas ? Psych Tech as available – will round a couple of times per shift and is always available if patient escalates ? Patient Ratios: o RN: Same on both shifts – acuity dependent ? Hem/Onc – 1:3 ? Observation/Off Service – 1:3-4 ? Bone Marrow Transplant – 1:1-2, based on acuity o CNA: 1:12 ? Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver’s License and/or Insurance: o BLS, PALS o APHON provider card ? APHON is just a certificate (like NIHSS) – CPHON and BMTCN are not required ? Skills required: o Required: ? Port access experience is a MUST, Bone marrow transplant experience with Day 0-2 is a MUST, Chemo administration, Blood product transfusion, cardiac rhythm interpretation ? Comfortability with blood cultures/central line care; PCA pumps, Heparin gtts o Preferred: ? End of Life care – Precedex gtt, Intransal Versed, Ketamine drips; chest tubes ?
