You will coordinate closely with physicians, therapists, social workers, and other team members to ensure that care is well organized and that patients have the resources they need to remain at home safely. You can expect to see a diverse adult and geriatric patient population with conditions such as post-surgical recovery, heart failure, COPD, diabetes, wound care needs, and rehabilitation after hospitalization. Patient visits are scheduled to allow for thorough assessments and meaningful education, rather than rushed encounters. Visit volumes and daily caseloads are structured to balance quality of care with manageable productivity expectations in the home health environment. RNs in this setting typically work day shifts with some flexibility to arrange visit routes across the Greensburg area and nearby communities.
