Documentation is completed in an electronic medical record system, with an emphasis on accurate functional reporting and outcome measures that reflect patient progress and regulatory requirements. Depending on the unit, patient caseloads are designed to support quality, individualized care and may range through a typical skilled rehab PT schedule over day or evening shifts. Shifts are most commonly daytime hours with potential for some weekend or holiday rotation to support continuous rehab services.
