In this position, you will provide one-on-one skilled physical therapy services in patients’ homes throughout the greater Pensacola area and surrounding communities. You will have the opportunity to build meaningful relationships with patients and families while helping them optimize function, independence, and safety in their own environment. This setting allows for continuity of care and holistic treatment, giving you the time to thoroughly assess, treat, and educate each patient. Typical responsibilities include comprehensive initial assessments, development and implementation of individualized plans of care, gait and balance training, strengthening and endurance programs, pain management strategies, home safety evaluations, and coordination of durable medical equipment needs. You will collaborate closely with nurses, other therapists, and home health aides to ensure that care plans are integrated and goal-driven. Documentation is completed using an EMR system, and you will be supported with clinical and operational resources to manage your caseload effectively. A typical day involves traveling between home visits within a defined territory, completing initial evaluations, follow-up treatments, and reassessments. Patient caseload is primarily adult and geriatric, often including individuals with orthopedic conditions, post-surgical needs, neurological diagnoses, cardiopulmonary disease, balance disorders, and generalized deconditioning. Visit volumes are structured to allow for high-quality, individualized care rather than rushed encounters, with a focus on functional outcomes and patient satisfaction.
