You will coordinate care closely with nursing and other therapy disciplines to help patients remain safely in their homes and reduce hospital readmissions. Visits are scheduled to allow for meaningful, unhurried patient interactions and thorough documentation. The typical schedule offers daytime hours with some flexibility to accommodate patient needs and personal preferences. Depending on agency needs, you may work full-time with a set weekly schedule, or in a PRN/part-time capacity offering additional flexibility.
