In this role, you will provide skilled physical therapy services to adult and geriatric patients in their homes throughout Roseburg and the surrounding area. Your typical day will include performing comprehensive evaluations, developing individualized plans of care, and delivering hands-on treatment focused on improving functional mobility, strength, balance, and safety in the home environment. You will educate patients and caregivers on home exercise programs, fall prevention strategies, safe transfers, and the proper use of assistive devices. You will manage a community-based caseload that may include patients recovering from surgery, stroke, cardiac events, fractures, and those living with chronic conditions such as COPD, heart failure, or neurologic diagnoses. The home setting allows for meaningful one-on-one treatment sessions tailored to the patient’s actual environment, giving you the opportunity to see the impact of your interventions on daily life.
