Your typical day will include traveling to patient homes throughout Eufaula and surrounding communities, completing visits that focus on functional mobility, ADL and IADL training, home safety and environmental modifications, adaptive equipment recommendations, and caregiver education. Caseload and visit volume are designed to allow you to provide thorough, high-quality care while maintaining appropriate documentation and communication with the broader care team. You will be expected to utilize an electronic medical record for timely and accurate documentation and to participate in case conferences and care coordination meetings. The position offers exposure to a diverse patient population in both town and rural settings, providing powerful experience in community-based rehabilitation and allowing you to see the direct impact of your interventions on patients’ daily lives.
