This Home Health Occupational Therapist position is based in Dover, Delaware, a capital city known for its friendly atmosphere, historic character, and convenient access to both coastal and metropolitan destinations. Dover offers a welcoming community with tree-lined neighborhoods, historic districts, and easy drives to Delaware’s beautiful beaches, wildlife areas, and state parks. Residents enjoy local festivals and events, museums and cultural sites, and a growing restaurant and shopping scene, all while benefiting from a relatively low cost of living and an easy commute within the region. Working in home health in Dover allows you to build meaningful, long-term relationships with patients in their own homes. You will serve a diverse adult and geriatric population, including individuals transitioning from hospital or rehab settings, those managing chronic conditions, and patients aiming to safely age in place. The typical caseload includes functional decline, fall risk, post-surgical recovery, neurologic conditions, and cardiopulmonary diagnoses, offering substantial variety and opportunities to deepen your clinical expertise. In this role, you will perform comprehensive evaluations in patients’ homes, focusing on safety, function, and independence. A typical day may include conducting home safety and environmental assessments, identifying fall risks, and recommending modifications such as grab bars, adaptive equipment, and layout changes to promote safe mobility and performance of daily tasks. You will develop and implement individualized plans of care that emphasize ADL and IADL training, energy conservation, cognitive strategies, and caregiver education tailored to each patient’s goals and living environment. You will collaborate closely with a supportive interdisciplinary team that may include physical therapists, speech therapists, nurses, social workers, and home health aides. Communication is primarily through EMR and regular case conferences, allowing for thorough coordination of care while preserving your autonomy in the field. Strong documentation skills and efficient use of EMR systems will be important, as you will complete evaluations, progress notes, and discharge summaries electronically, often at or near the point of care. Patient volume and visit frequency are structured to allow time for individualized, high-quality interventions. Visit schedules typically include a mix of start-of-care evaluations, routine follow-ups, reassessments, and discharges. The number of visits per day will depend on patient needs, acuity, and travel distances within the Dover area, with careful planning to optimize route efficiency and minimize windshield time. You can expect regular daytime hours with some flexibility in scheduling to accommodate patient availability, with opportunities for occasional evenings or weekends as required for continuity of care. This position offers the chance to work with patients in a variety of home settings, from historic residences in central Dover to newer suburban developments and rural properties in the surrounding area. These environments provide rich opportunities to apply practical, real-world problem-solving and creative home modifications to maximize safety and independence. As you help patients remain in their homes safely, your work will have a direct and visible impact on their quality of life and on caregivers’ confidence and capacity. The professional culture emphasizes collaboration, clinical excellence, and ongoing learning. You will be supported in keeping your skills current through access to continuing education, mentorship from experienced clinicians, and exposure to a diverse caseload that can strengthen your clinical judgment and broaden your experience. The role is well-suited for Occupational Therapists who value autonomy, community-based practice, and meaningful patient relationships, while enjoying life in a city that blends historic charm with modern conveniences and easy access to outdoor recreation.
