You will travel to patient homes throughout Kirkland and nearby communities, using your clinical skills to help individuals regain independence and safely perform everyday activities in their own environment. Your day-to-day work will involve managing a field-based caseload, traveling between homes, completing timely EMR documentation, and communicating with the broader care team. You will work within established home health guidelines and regulatory standards, ensuring accurate OASIS or other required assessments, and adhering to infection control and safety protocols in diverse home environments.
