POSITION SUMMARY – RN Case Manager POSITION DUTIES – Responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice. Provides services utilizing nursing theory and processes, communicates/documents observations and assessments, promotes and maintains health of individuals through direct care, teaching, counseling and appropriate rehabilitative measures. MINIMUM REQUIRED QUALIFICATIONS – MA RN license; BLS; 2 years acute case management experience LENGTH OF ASSIGNMENT – 26 weeks SHIFT / HOURS PER WEEK – M-F 8am-4:30pm with weekend rotation of 1/4 weekend requirement START DATE – ASAP
