Day-to-day responsibilities include conducting evidence-based assessments, documenting thoroughly and efficiently in the electronic medical record, participating in team rounds or care conferences, and providing timely education to patients, caregivers, and staff. You will play a key role in recommending safe diet levels and compensatory strategies, making instrumental assessment recommendations, and contributing to discharge planning to support safe transitions of care. The position is structured as a full-time, day-shift schedule, working five 8-hour shifts per week for a total of 40 hours.
