As a traveler in this home health OT role, you will provide one-on-one, patient-centered care in the comfort of patients’ homes across the valley and surrounding rural areas. You can expect a caseload primarily consisting of adult and geriatric patients with a range of diagnoses, including post-orthopedic surgery, neurologic conditions, cardiopulmonary disease, deconditioning, and chronic illness management. Typical responsibilities include initial and follow-up assessments, development and progression of individualized care plans, home safety and fall risk evaluations, ADL and IADL training, energy conservation strategies, adaptive equipment and DME recommendations, and caregiver education. A typical day includes driving between patient homes throughout the Santa Ynez Valley, managing a schedule of several visits per day depending on evaluation versus treatment mix. Visit types may include start-of-care assessments, routine treatment visits, recertifications, and discharges. You will collaborate with an interdisciplinary home health team that may include nurses, physical therapists, speech therapists, medical social workers, and home health aides. Communication is generally coordinated via phone, secure messaging, and EMR documentation, with supportive clinical leadership to assist with complex cases. Shifts are generally daytime, Monday through Friday, with some flexibility to accommodate patient and therapist scheduling needs. Weekend or evening work, if needed, is typically limited and planned in advance. Documentation is completed electronically, and you will be expected to manage your time effectively to balance travel, patient care, and timely charting.
