Pharmacists in this long-term acute setting play a critical role in therapeutic drug monitoring, particularly for medications with narrow therapeutic indices. You may oversee vancomycin and other high-risk medication levels, adjust dosing based on lab results and patient response, and ensure appropriate lab follow-up. Antimicrobial stewardship is a key aspect of the role, and you will help guide appropriate antibiotic selection, de-escalation, and duration of therapy in order to balance effectiveness with resistance prevention. Medication safety and quality improvement are integral to the position.
