You will collaborate closely with physicians, therapists, social workers, and home health aides to coordinate care and ensure that each patient’s needs are met. Your day will typically involve traveling between several patient homes, performing visits that include assessment, intervention, and documentation. Patient caseloads and daily visit expectations are designed to allow thorough, quality care and timely documentation. You can expect to see a variety of conditions, including post-acute surgical patients, those with cardiopulmonary diseases, diabetes, neurological conditions, and patients requiring palliative support. You will use an electronic medical record system for all documentation and communication.
