On a typical day, you will provide direct patient care under the supervision of a licensed Physical Therapist, implementing individualized treatment plans for residents in the SNF as well as patients scheduled in the outpatient clinic. In the SNF portion of your day, you can expect to assist with bed mobility, transfer training, gait training with assistive devices, fall-prevention programs, and functional activities of daily living to promote safe discharge back to home or community settings. You will work with an interdisciplinary team to support rehabilitation after joint replacements, fractures, strokes, and other complex medical conditions, contributing to weekly or biweekly care conferences. In the outpatient setting, you will help treat patients with orthopedic, post-surgical, and chronic musculoskeletal conditions.
