This Certified Occupational Therapy Assistant position is based in a skilled nursing facility in Grants Pass, Oregon, a vibrant small city in southern Oregon known for its friendly community, scenic beauty, and relaxed lifestyle. Grants Pass sits along the renowned Rogue River, offering easy access to fishing, rafting, kayaking, hiking, and year-round outdoor recreation. The surrounding area features lush forests, nearby mountains, and convenient access to regional parks, wineries, and charming downtown shops and restaurants. It provides an appealing mix of small-town feel with the amenities you need, all in a setting known for mild seasons and abundant sunshine for the Pacific Northwest. The skilled nursing facility offers a supportive environment focused on quality post-acute and long-term care. You will join an interdisciplinary team that includes occupational therapy, physical therapy, speech therapy, nursing, social services, and activities staff, all dedicated to helping residents regain function, maintain independence, and improve quality of life. The building features dedicated therapy spaces and equipment designed to simulate real-life tasks and environments, helping patients prepare for a safe transition back to their homes or to a higher level of independence. As a COTA in this setting, you will work under the direction and supervision of a licensed Occupational Therapist. Your typical day includes assisting with evaluations through data collection, implementing established treatment plans, and providing one-on-one and group interventions focused on activities of daily living, functional mobility, upper extremity strengthening, balance training, and cognitive support. You will help residents practice self-care tasks such as dressing, bathing, grooming, and safe transfers, and you will train them in the use of adaptive equipment and compensatory strategies. You will also contribute to documentation in the EMR, communicate daily with the supervising OT about patient progress, and participate in care conferences as appropriate. Patient caseload is primarily geriatric, including individuals admitted for short-term rehabilitation after surgery or hospitalization, as well as long-term residents who require ongoing support to maintain function and engagement in daily routines. Diagnoses may include orthopedic conditions, strokes, generalized deconditioning, neurologic disorders, cardiopulmonary conditions, and progressive illnesses common in older adults. The facility’s therapy and nursing teams collaborate closely to address complex needs, manage fall risk, and promote safe discharge planning when appropriate. Shifts are typically daytime hours with some flexibility based on facility needs and patient census. Weekend and holiday rotations may be required to ensure consistent therapy services for residents. Productivity expectations are designed to be reasonable and achievable while still allowing time for quality patient interactions and accurate documentation. This role is a strong fit for a COTA who values meaningful patient relationships, appreciates the opportunity to follow patients through different stages of recovery, and enjoys working in a team-oriented, post-acute environment. It is also ideal if you are drawn to the lifestyle that Grants Pass offers: close proximity to the Rogue River, outdoor adventures just minutes from home, and a welcoming community that supports both professional and personal growth.
