Weekly Pay*
The Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education. Evaluates and establishes plan of care to meet patients’ needs in the home environment. Coordinates care with other disciplines and seeks orders for appropriate additional services. · Provides direct patient care based on the established care plan and physician orders. · Applies knowledge of physical therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation in evaluations, re-evaluations, and treatment techniques. Modifies treatment as appropriate, including frequency and duration of care. · Masters scoring of the CMS required OASIS and works with Quality team to collaborate needed adjustments. · Acquires and maintains a basic understanding of the insurance requirements in Home Health. Qualifications include BS degree or better from an accredited Physical Therapy program with current license to practice in the State of Montana, valid driver’s license, and appropriate auto insurance. Two years of previous PT experience preferred. BLS
