You will design and implement individualized treatment plans focused on ADL and IADL training, functional mobility, upper extremity function, cognitive and perceptual skills, and adaptive equipment or environmental modifications. Education of patients and caregivers is a key part of the role, along with thorough documentation in the EMR and participation in interdisciplinary care conferences as needed. Depending on the specific assignment and setting, caseload may include a mix of outpatient and inpatient or post-acute rehabilitation patients.
