  • Assists members with housing issues.
  • Provides accurate and timely documentation according to departmental guidelines. Enters running notes in medical charts. Maintains accurate records on Social Work/patient activities.
  • Develops knowledge of community networks and resources for elderly, developmental or physically disabled population of diverse ethnic and cultural backgrounds and forms collaborative relationships with same. Provides information to members to assist in establishing and maintaining community links.
  • Conducts biopsychosocial assessments, providing individual and group counseling, supporting behavioral health interventions, and ensuring compliance with CMS and DHCS requirements for timely interdisciplinary team (IDT) care planning.
